Mungabareena Aboriginal Corporation is seeking a dedicated Pathways to Good Health Coordinator.
The Coordinator role plays a vital role in supporting the Pathways to Good Health Project. The primary responsibility is to foster the development and sustainability of partnerships and collaboration within internal programs and external services such as ACCO's, ACCHO's, child protection services, and broader community services, ensuring the triage and ongoing delivery of high-level patient care for both children and their carers.
This identified position is open to Aboriginal and Torres Strait Islander applicants and offers the opportunity to contribute to the strength and self-determination of the local Aboriginal community.
In this key role, you will:
- Engage with and access ACCO and partnership systems, ensuring seamless connections with internal units, teams, and external organisations, including both other ACCO's and mainstream or community services.
- Facilitate access to relevant health and well-being specialists and service supports for children, ensuring a comprehensive person-centred care approach.
- Oversee the implementation of culturally safe and responsive health pathways for Aboriginal and/or Torres Strait Islander children in care, maintaining strong communication between service providers and families.
- Collaborate with service partners to continually review and refine care strategies, addressing any barriers to accessing health and wellbeing services.
- Act as an interface between the Child Protection workforce and health services, ensuring cross-sector collaboration, providing health service navigation, education, and capacity building.
- Conduct referral triage, management, and case allocation, ensuring timely facilitation of multidisciplinary assessment and referral to other specialty health services such as early intervention as appropriate.
- Contribute to the development of evidence-based health and SEWB initiatives for children in care, encompassing models of care and frameworks that empower individuals, families, and communities while respecting their cultural contexts.
This position requires willingness to provide, complete, and submit evidence prior to commencement of employment of:
- A current Driver's Licence.
- A current Victorian Working with Children Check.
- A current Police Check.
