Nurse Manager - Home Services page is loaded## Nurse Manager - Home Serviceslocations: Geelong, Victoriatime type: Part timeposted on: Posted Todaytime left to apply: End Date: September 12, days left to apply)job requisition id: JR-12700**Your role at St John of God Healthcare at Home**We are seeking a dynamic, highly motivated and enthusiastic **Nurse Manager** to provide leadership and oversight of our service in **Geelong**.This is an opportunity to lead a service with significant growth and a thriving new home maternity service, working collaboratively with our hospital and external providers to deliver a first-class service.You will provide management of the service as well as deliver direct patient care, as required, to ensure a positive patient experience and clinical outcomes in line with the Home Services Strategy.We are offering a permanent position working **45 hours** per fortnight.**The Position:*** Leads and fosters a culture of continuous improvement where all patient encounters influence a memorable patient experience, and which is underpinned by excellence and safety in care.* Monitors, analyses, and reports variances and trends in clinical data to continuously improve clinical quality and safety outcomes.* Contributes to policy and procedure development to optimise clinical outcomes and enhance the patient experience.* Actively promotes and integrates service models with the hospitals to deliver seamless care in the community and ambulatory setting.* Promotes the delivery of evidence-based practice, including research and quality activities to improve patients' care in the home setting.* Participates in, contributes to and implements quality improvement and risk management into all aspects of service.Successful applicants will be registered as a Registered Nurse with the Nursing & Midwifery Board of Australia (AHPRA), hold a Tertiary qualification in nursing, management and/or relevant discipline, and have recent acute clinical experience in home nursing.
