About the Role
We currently have an exciting opportunity for an experienced Case Manager to join our passionate team at Maitland Community Care Services, supporting clients with Home Care Packages in a full-time role.
At ADSSI, we understand the importance of work-life balance. After completing your onboarding and gaining confidence in the role, you'll typically have the flexibility to work from home up to two days per week. The specific days and frequency of remote work can be adjusted in line with business needs and service requirements.
As a Case Manager, you'll play a key role in delivering high-quality, person-centred services; coordinating support that enables clients to live safely and independently at home.
Key Responsibilities
- Conduct home visits, client assessments, and ongoing case management
- Develop, implement, and monitor support plans aligned with client goals and preferences
- Coordinate resources and services to ensure high-quality care
- Manage client budgets and service agreements within funding guidelines
- Liaise with brokerage and allied health services as needed
- Maintain accurate records and provide regular reports
- Ensure services are delivered in compliance with relevant policies and timeframes

About You
- A Diploma in Community Services, Case Management, is desirable.
- Proven experience in case management and care coordination for older people, people with disability, and people living with dementia and their carers is essential.
- Sound knowledge of Consumer Directed Care Home Care Package guidelines.
- Experience working within client budgets and achieving service delivery outcomes.
- You are professional and collaborative in your approach, with a strong commitment to working as part of a team. You bring excellent attention to detail, are highly self-motivated, and thrive in a supportive, with a focus on empowering choice.
- A current NSW Driver Licence and a safe driving record
- A current or willingness to obtain: NDIS Worker Check & Working with Children Check
