An exciting opportunity is available for a Client Coordinator to join our Brightwater At Home Transitional Care Team, which provides short-term, goal-oriented care for 12 weeks that seeks to optimise the designed to optimise the functioning and independence of older people after a hospital stay.

This is a full-time, Monday to Friday position, working in the community across the North Metropolitan area.

With over 100 years of care experience, Brightwater is one of the country's leading care providers. We are committed to helping people live with dignity and independence and we're looking for a dedicated Client Coordinator to help us deliver on that mission.

This role is ideal for someone with experience in health, aged care, or community services who wants to make a real difference in people's lives. It may also appeal to Registered Nurses who are seeking a new direction where their clinical skills are valued and applied in a different way.

About the Role

Our Transitional Care Program supports clients for up to 12 weeks after hospital, helping them to regain independence and confidence at home.

As a Client Coordinator, you will:

  • Work as part of a multidisciplinary team to assess, plan, and coordinate tailored services.
  • Support older people to optimise their independence and quality of life in their own homes.
  • Apply your knowledge, organisational skills, and problem-solving in a role that blends care, coordination, and empowerment.

To be suitable for this role, you must possess:

  • Significant experience in a health, aged or community service environment with an understanding of home care packages, CHSP guidelines and Aged Care Quality Standards
  • Excellent written, verbal and interpersonal communication skills with the ability to inclusively communicate with a diverse range of people, building and developing purposeful relationships and collaborative teamwork.
  • Excellent customer service focus and strong person-centred skills with the ability to effective engage clients requiring services.
  • Demonstrated ability to organise and prioritise tasks, work effectively under pressure, problem solve and meet deadlines.
  • Current Western Australian 'C' Class driver's license
  • National Police Certificate (obtained within the last 12 months).
  • Working knowledge of occupational safety and health legislation; Aged Care Act (1997); Aged Care Quality Standards (desirable).
  • Experience using client management software systems and experience with case management (desirable).
  • Clinical/Health Experience i.e Social work/Nursing (desirable).
