This is a Fixed Term Contract until July 2028, based in the Hunter Region.

Responsibilities

The Dementia Support Coach will provide on-the-ground support, implement relationship-based support strategies, and model and coach clients, carers, and care staff.

  • Collaborate with peers to develop the skills, expertise, and applied knowledge of aged care staff and carers to understand and support people living with dementia effectively.
  • Support the implementation of recommendation plans through modelling and coaching, seeking guidance from team leaders and consultants as needed.
  • Facilitate and support the trial and implementation of brokerage items with clients and their carers, with guidance from supervisors.

What We Need From You

  • Experience supporting individuals living with dementia in residential and community aged care settings
  • Strong organisational skills, including effective time management and prioritisation in a busy environment
  • Ability to work independently and as part of a team
  • Commitment to customer service principles, conflict resolution, and providing exceptional service
  • A current driver's licence and safe driving record
  • Willingness to travel intrastate and interstate to support DC services
  • Relevant qualifications such as Allied Health Assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health, or similar (highly desirable)
