Senior Care Professional

Provide high-quality care to individuals with dementia in our transitional care program.

  • Work as a team member, providing tailored individual care to each resident, including personal and domestic support within the residential cottage.
  • Collaborate closely with experts, including Psychogeriatrician, Clinical Nurse Consultant, Program Manager, and Dementia Advisors.
  • The HammondCare model prioritises residents' needs, creating a home away from home.
  • Ongoing training and support are provided through onsite CNC, SDCP educator, Dementia Centre experts, and Workplace coaches.
  • Participate in comprehensive weekly reviews of residents and develop behaviour support plans and strategies.

To be successful in this role:

  • Hold unlimited work rights as a permanent resident or Australian Citizen.
  • Have a Certificate III or Certificate IV in Aged Care.
  • Experience in dementia care, NDIS, or complex needs is essential.
  • Be passionate about serving elderly people living with dementia and want to make a difference in their lives.
  • Value teamwork but also be able to work independently and show initiative.
  • Be available to work mornings or afternoons on a part-time basis.
  • Have knowledge and experience caring for people living with dementia and their needs.
