About The Role:
- Clinical in-home services include care plan assessment, medication, incontinence, wound, chronic disease, complex wound management, palliative care, health and diabetes management
- Clinical delivery and supervision of non-clinical team members in a community setting
- Intake processing, clinical care planning and reporting
- Conducting client care assessments with a focus on Consumer Directed Care
- Coordinate the provision of quality person-centred care according to the client's needs, service/care plans and available funding according to the client's budget
- General administration for documentation relating to client care and in alignment with AHPRA regulations and legislation
Qualifications
- Qualified Enrolled Nurse
- Current AHPRA registered
- First Aid & CPR
- Australian National Police check
- Professional Indemnity Insurance
- Experienced in assessing and planning care needs within the community
- Valid Driver's Licence and reliable presentable car
- A smart mobile phone (iphone / android )
Preferred Skills
- A passion for high quality client-focused care in a community setting
- Cert IV in Training and Assessment - highly desirable
- Exceptional communication skills
- Able to work autonomously or within a team environment
- Excellent time management and organization skills
- Commitment to ongoing professional development
- Residing in Western Australia is a requirement
