The Opportunity

The Role

The Team Leader ensures that the Dementia Support Australia (DSA) service is tailored to individual client needs and meets its key performance indicators.

This role is a Temporary Full Time or secondment opportunity until July 2028, supporting our Tasmania HACDSP Team.

Responsibilities
  1. Provide leadership to a multidisciplinary team across up to three locations (Hobart, Launceston, and referrals from North West Tasmania), focusing on the DSA Program, in line with policies, procedures, and HammondCare values.
  2. Work with an integrated team to ensure a consistent approach to our 'Next Chapter' initiative.
  3. Develop a culture of empowerment, mentorship, and support for the team, ensuring wellbeing and safety.
  4. Oversee service delivery to operate within the case management framework, reducing the impact of behaviours on people with dementia, their carers, and staff.
  5. Collaborate with other Team Leaders to meet broader program goals and deliver solutions.
  6. Contribute to the program's administrative, financial, risk, quality, and reporting objectives.
  7. Provide direct support and consultation to clients as needed.
  8. Ensure timely referral allocation to DSA staff aligned with KPIs.
  9. Report regularly to the Regional Manager on staff performance, service quality, and operational issues.
  10. Recruit suitable Dementia Consultants, Support Coaches, and Family Liaison Officers.
  11. Manage stakeholder relationships with local managers and staff in aged care, home care, and hospitals to promote program understanding and effectiveness.
  12. Handle customer complaints or challenging situations promptly, notifying management when necessary.
  13. Address incidents and feedback related to service quality with the Regional Manager.
  14. Enhance personal knowledge and skills through ongoing learning.
  15. Ensure a safe office environment, including maintenance and equipment management.
  16. Oversee the use and maintenance of fleet vehicles, including tracking and reporting.
  17. Manage approvals for brokerage and travel in accordance with financial policies.
What We Need From You
  • Commitment to HammondCare's Christian Mission, Motivation, and Action.
  • Qualifications in Nursing, Allied Health, or a related health degree.
  • Experience supporting older people and carers with dementia, emphasizing psycho-social case management approaches.
  • Ability to lead teams across multiple sites.
  • Strong multitasking, communication, planning, and organizational skills.
  • Attention to detail and analytical skills.
  • Customer management and administrative skills.
  • Willingness to travel intra- and interstate.
Desirable
  • Eligibility for Australian Health Practitioner registration.
  • Qualifications or studies related to dementia.
