As a Senior Personal Carer you'll conduct initial client intake and screening assessments and develop and review individualised care plans with a client-centered approach while providing quality care based on clients' needs.

• Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
• Management of client information in CRM & CMS ensuring all details and case notes are kept up to date.
• Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team as required.
• Spend time on the road completing scheduled visits, interacting one-on-one with clients within their homes, supporting daily activities.
• Provide care in accordance with the individualised Care Plan.
• Support community-based recreational and social activities.
• Provide transportation and assistance with shopping and appointments.

• Certificate III Aged Care, Individual Support or an equivalent qualification – Essential
• Current Australian Driver's Licence (Green Provisional minimum) – Essential
• Roadworthy & insured (Third Party Property minimum) vehicle to use for work – Essential
• Experience with technology incl. smartphones, tablets, laptops etc
• Confidence to satisfy the relevant probity checks required by legislation or policies
