This is a truly exciting time to join the Lincolnshire Stroke Service (LSS) as we have an exciting opportunity for a dynamic, ambitious, and forward-thinking Physiotherapist to work if interested across acute and community settings. We are seeking a Physiotherapist with demonstrable experience within the Neurological field, willingness to learn and passion for improving the lives of people who have recently had a stroke. This is a rare opportunity to learn and develop from specialists within an acute hospital and community team.

LSS has a seamless pathway from an acute inpatient setting to community delivery and both services work collaboratively across the patient pathway. The acute element of stroke delivery has been centralised on a single site (Lincoln County Hospital, LCH) whilst the community arm of the pathway is delivered countywide with four bases.

The post will be hosted by Lincolnshire Community Health Services (LCHS)so the ability to travel is therefore essential. The potential rotations between other neuro services can be discussed further at interview and we would welcome applications from candidates based across Lincolnshire. This post would include being part of the therapy weekend cover rota.

This post will require a full driving licence and access to a vehicle to travel.

Main duties of the job

LSS prides itself on delivering the highest quality of care to patients in the right setting, at the right time, by the right clinician. You will play a key role in delivering patient-centred rehabilitation and be afforded opportunities to work with patients at different stages of their stroke recovery. You will be a core member of the wider multi-disciplinary team which include consultants, nurses, clinical psychologist, specialist physiotherapists, speech and language therapists and occupational therapists, assistant practitioners and rehabilitation assistants.

You will work as an autonomous practitioner, undertaking specialist assessment and treatment which is evidence-based, and delivering this efficiently and effectively with regards to clinical priorities and the use of time. You will agree individual rehabilitation programmes based on patient-led outcomes, utilising both your skills and that of the wider multi-disciplinary team to achieve maximum potential.

This role also affords the opportunity to assist the Physiotherapy Leads / Team Leads to develop and maintain a service structure that delivers best practice, assist in the development of training, guidelines and innovations which may benefit the service. As a valued member of the team, you will have the opportunity to develop your skills both clinically and from a leadership perspective benefitting from expertise within both the acute and community teams.

Job responsibilities

This post is unable to support applications from those individuals requiring sponsorship to work in the UK

  • To provide the specialist physiotherapy element in the Lincolnshire Stroke Service, including providing clinical specialist support, professional advice and practical guidance across Acute and Community stroke pathways.
  • To act as case manager to a designated case load which will include ensuring all patients are able to access all elements of their care plan and to ensure the individual care plans are delivered and progressed by the team.
  • To manage a clinical caseload to a high professional standard and provide effective evidence based clinical service for patients as an autonomous practitioner. This will include more advance & detailed assessments with patients having more diverse & complex conditions, providing a diagnosis and develop and deliver an individualised treatment programme to neurological patients, as part of a specialist programme of intervention.
  • To ensure a high standard of care for all patients by effective communication with patients, carers, other team members and the wider system partners.

Person Specification
  • Car Driver with valid UK driving license
  • Relevant VISA in place/Right to work in the UK
Qualifications
  • Degree or equivalent postgraduate diploma in Physiotherapy
  • Current Health Care Professions Council Registration
  • Evidence of postgraduate training in stroke rehabilitation
  • Member of Chartered Society of Physiotherapy
  • Member of a related specialist physiotherapy group
Skills and competencies
  • Good communication skills both verbal and written
knowledge and experience
  • Experience of working in the NHS as a Physiotherapist
  • Experience as a physiotherapist with evidence of experience and training in stroke rehabilitation.
  • Specialist knowledge gained through post graduate clinical experience and specialised post graduate training and courses.
  • Experience in supervision
  • Knowledge of the workings of primary care, secondary care, community services and intermediate care services.
