Health Access and Equity Coordinator

Brisbane South PHN has an exciting opportunity to become part of our dynamic System Integration and Coordination team. With strong project management experience in health, you will support Brisbane South PHN in planning and responding to health system reform, strategic priorities and identified gaps in our community. This is a full-time fixed term role to 18 December 2026.
The objective of this position is to:
- Support the successful implementation of The PRIDE Co-Design Project by coordinating the recruitment and engagement of 20 x primary healthcare clinical sites and providing quality improvement (QI) coaching and support to embed an LGBTIQA+ affirming Model of Care into mainstream primary care settings.
- Contribute to existing new and emerging health access and equity initiatives in collaboration with PHN teams, key partners, and sector stakeholders.
