Right at Home is hiring a passionate and experienced Care Coordinator

Right at Home Sydney Inner West is looking for a qualified and experienced Care Coordinator (Full Time) for our in-home care services. The ideal candidate will demonstrate client-centered practice, empathy, dignity, and respect. You will be a natural relationship builder, honest, helpful, and dependable, making a valuable addition to our team.

This role involves:

  • Meeting with clients and assessing their needs.
  • Preparing in-home care assessments and care plans.
  • Providing in-home care services as required.
  • Handling inquiries, feedback, and complaints.
  • Training and managing the care team's performance.
  • Ensuring all documentation is accurately completed in our databases and software.
  • Providing after-hours "on-call" support on a rotating schedule.
Qualifications
  • Minimum Cert 3 or 4 in individual support for Ageing or Disability (ESSENTIAL)
  • Experience in case management or care coordination with a Home Care Package.
  • Current Australian Police Check & Working with Children Check.
  • Current First Aid & CPR certification.
  • Driver's license and own reliable, registered vehicle (Essential).
  • Knowledge of the Home Care industry and Aged Care Standards is highly regarded.

Job Types: Full-time, Permanent

Pay: $80,000.00 – $90,000.00 + superannuation per year + travel allowance
