About the Care Finder program
Here at Each, we are looking for a Team Lead to join our Care Finder program, which aims to provide specialist and intensive assistance to help people in the Care Finder target population groups understand and access aged care services and connect with other relevant supports in the community.
- Part-time (0.8FTE) ongoing position (Monday - Thursday)
- Based at our Coffs Harbour office with travel required. Hybrid working conditions.
What you'll be doing
In this fast-paced role, no two days will be the same As Team Lead, your duties will include:
- Providing senior consultation, line management, and support to the Care Finder team, who practice assertive outreach and explore different ways to effectively engage with people in the Care Finder target populations.
- Assisting with the development, implementation, and evaluation of a robust service program.
- Managing a 50% caseload of clients, aligned with your commitment to customer care.
- Providing specialist and intensive assistance to help senior Australians who need support, to understand and access aged care services and connect with other relevant supports in the community.
- Building and establishing relationships within the community to promote the Care Finder program and reach potential clients.

If you're a born leader with a commitment to delivering a person-centred approach, and an understanding of the policy and funding environment of the aged care and community services sector, then you might be exactly who we're looking for
Experience, Knowledge, and Qualification/s required for the job
- Qualification in Social Work, Human Services, Aged Care, Community Services, or Health, and/or relevant experience, with a passion for assisting older adults in our community.
- Demonstrated experience in leading and managing a team.
- Experience working with older adults within the community, with an understanding of local supports and services.
- Exceptional interpersonal, communication, negotiation, and problem-solving skills.
- Ability to work remotely with a team when required.
- Current NSW driver's license and vehicle.
- Computer literacy and ability to manage a client database effectively.
