Job Title: Home Care Professional

As a dedicated and compassionate healthcare professional, you will play a pivotal role in delivering exceptional patient care and support within the comfort of their own homes.

About the Role:
  • This is a casual opportunity based in the East Perth Metro area.

You will provide effective leadership, supervision, and clinical support to our customers as required, ensuring that they receive the highest standard of care possible.

Your Responsibilities:
  • Travel between customer's homes and provide individualised and personalised support to customers in accordance with their support plans and/or medical practitioner's directions.
  • Work under the guidance and supervision of medical practitioners, collaborating with allied health professionals, monitoring and communicating the customer's condition, and other health and wellness-related activities.
  • Demonstrate a commitment to best practice, ensuring that all services are provided in a caring and respectful way in accordance with legislative requirements.
  • Administer medication and provide complex clinical care, including palliative care, bowel care/suppositories, catheter care, stoma care, oxygen, and more.
  • Monitor and communicate on the customer's wellbeing and report on progress of customers.
  • Ensure that all services are provided in a caring and respectful way in accordance with legislative requirements and program-specific guidelines.
  • Provide training, coaching, and guidance to Community Support Professionals, Enrolled Nurses, and Support Staff/Case Managers as required.
  • Conduct skills assessments of Community Support Professionals performing complex care services as required.
  • Demonstrate a commitment to best practice and evidence of continued professional development.
  • Participate in staff meetings and attend training opportunities when required.
About You:
  • You will have demonstrated high-level verbal and written communication skills.
  • You will have proven relationship management with all internal and external stakeholders, as well as good interpersonal, negotiation, and problem-solving skills.
  • You will be able to work as part of a team.
  • You will be comfortable using technology for accessing rosters & recording customer assessments & documenting feedback.
Requirements:
  • You must have current registration with Australian Health Practitioner Regulation Agency (AHPRA).
  • You must provide evidence of continued professional development.
  • You must undergo a Criminal Record Check.
  • You must have a Current CPR / First Aid Certificate.
  • You must have a Current Australian Driver's Licence.
  • You must have a reliable, registered, and insured vehicle (comprehensive insurance is preferred).
  • Prior experience in the community care or health/aged care sector is preferred.
  • You must provide proof of vaccination (where relevant), i.e. COVID-19 vaccination certificate.
