What you'll be doing

The nursing vision for Prince of Wales Hospital is Transforming Person-Centred Care Together. As a Clinical Nurse Consultant in SESLHD you will play a pivotal role to Prince of Wales Hospital in clinical service and consultancy, clinical leadership, research, education and clinical services planning and management. This is a unique chance to combine clinical expertise with groundbreaking research, contributing to advancements in cardiology while working alongside a supportive and innovative team.

Key Responsibilities
  • Long Length of Stay (LOS) Meetings: Lead and participate in multidisciplinary meetings to review and manage patients with extended hospital stays, aiming to identify and address barriers to timely discharge.
  • Key Performance Indicators (KPIs): Monitor and report on clinical performance metrics, implementing strategies to achieve and exceed established KPIs.
  • Complex Discharge Planning: Develop and oversee comprehensive discharge plans for patients with complex needs, ensuring safe and effective transitions from hospital to home or other care settings.
  • Family Meetings: Facilitate discussions with patients and their families to provide information, address concerns, and support decision-making regarding care and discharge plans.
  • Interfacility Transfers: Coordinate the transfer of patients between healthcare facilities, ensuring all clinical and logistical aspects are managed effectively to maintain patient safety and care continuity.

This position is classified as Grade 2 under the NSW Public Health System Nurses' and Midwives' (State) Award, reflecting the advanced practice and leadership responsibilities inherent in the role.

Selection Criteria
  • Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse
  • Clinical Nurse Consultant/Clinical Midwife Consultant Grade 2 Having at least 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field, preferably in an acute setting and experience in patient flow and a community setting. In addition, must have approved postgraduate nursing qualifications relevant to the field in which he/she is appointed, or such other qualifications or experience deemed appropriate by the public hospital or public health organisation. An employer may also require a higher qualification in the specialist nursing field where such a qualification is considered essential for the performance of the individual position.
  • Demonstrated ability to apply clinical expertise in a consultant role and the ability to work independently.
  • Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
  • Demonstrated evidence of the use of critical thinking as it relates to the delivery of advanced evidenced based nursing care.
  • Demonstrated experience in leading and implementing change, and developing direction in clinical practice by initiation, utilisation, review and translation of evidence-based research into practice, within a person-centred framework.
  • Demonstrated effective interpersonal skills, excellent written and verbal communication skills, advanced computer skills and use of software programs to analyse data and produce reports.
  • Demonstrated ability to contribute to strategic and clinical service planning aiming at reduced hospital length of stay, alternate care pathways and experience in navigating complex discharges and family dynamics.
