Aged Care Case Manager
We are currently looking for an Aged Care Case Manager for a newly registered Aged Care provider.
The business has all required systems, policies & procedures, staff and documentation but requires experience to assist with launching, initial set up, client attraction and on-boarding of clients.

As the Aged Care Case Manager, you will be integral in the launch and initial operations with responsibilities including:
- Review of business documentation, policies & procedures and final set up advise
- Business development and client attraction
- Initial client assessment, planning, agreements and on-boarding
- Develop, implement, evaluate, and regularly review client-cantered care plans
- Work closely with roster team to coordinate the delivery of quality care
- Oversee the financial management of care planning to ensure services are delivered according to legislation and within allocated budget
- Maintain up to date client records of assessments, care plans and services
- Be involved with client changes and incident reports
- Providing advice and expertise on ways to improve services and quality
- 5+ years Case Management experience (experience with start-up or small providers highly regarded)
- Clinical background of AIN, EN, RN or Allied health highly regarded
- A thorough knowledge of Aged Care standards and legislation
- Prior experience with signing up and on-boarding clients
- Proficiency in conducting assessments, care planning, implementation, evaluation, and documentation of quality client care in line with Consumer Directed Care principles
- Strong communication skills and ability to build rapport with clients from various backgrounds
- Current police check
- Working With Children Check or willingness to obtain
- Current First Aid and CPR
- Up to date Flu and COVID-19 Vaccinations
- Unrestricted Australian working rights
- Full Drivers License and reliable vehicle
