  • Reason for vacancy: OT staff member departed for a new role at Townsville University Hospital
  • Caseload: 90% Transition Care Program in Townsville suburbs; 10% NDIS caseload from our Townsville office clinic rooms
About the Transition Care Program (TCP):
  • The service is the lead allied health provider for the program within the region
  • Established in 2005, the TCP is a joint Commonwealth and State-funded initiative supporting older Australians in regaining physical and psychosocial function after hospital stays. Delivered nationally through State and Territory Health Departments, the program offers short-term restorative care, up to 12 weeks, to enhance independence and prevent premature residential aged care entry.
  • The program is goal-oriented, time-limited, and therapy-focused, with services tailored to each individual. These include nursing support, personal care, domestic assistance, transport, and low-intensity allied health therapies such as Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, and Social Work.
  • In our region, TCP offers 46 community-based places servicing Townsville, Magnetic Island, Ingham, Charters Towers, Ayr, Home Hill, and Mt Isa.
  • Experience required: minimum 2 years; there are 4 other OT team members based in Townsville

  • Driving license required: Yes, as TCP is a home-based community health service
  • Accommodation: available and can be organized if needed
  • Relocation support: available, including flights and petrol
