About The Role

We are seeking a dedicated Social Worker to join our palliative care team In this role, you will work closely with the palliative nursing team, providing psychosocial interventions and support to individuals on their palliative journey.

Key Responsibilities Include
  • Conducting psychosocial assessments
  • Providing individual, couple, and family support
  • Leading group work, consultation, and education
  • Undertaking home visits as part of community-based care
This role is well-supported by the Lower Hunter Head of Social Work and Nursing Manager, with direct reporting to the Head of Social Work. Monthly supervision is a requirement, and ongoing professional development is encouraged and supported through the Lower Hunter Allied Health Department.

Where You'll Be Working

The Maitland Dungog Palliative Care Service is seeking a dedicated professional to join our multidisciplinary team based at the High Street Campus (formerly the Old Maitland Hospital). As part of the Maitland Kurri Kurri Hospitals network, our community-based service works closely with local agencies to provide compassionate support to patients, families, and friends on their palliative journey.

About You

We are seeking a dedicated and independent Social Worker to join our team. The ideal candidate will have experience in therapeutic counselling and group work, with the ability to work autonomously without direct supervision. A valid driver's license is required, as travel across the region will be necessary to support clients. Additionally, participation in an after-hours social work roster may be required.

Requirements
  • Must hold as a minimum a bachelor degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer.

  • To be eligible for permanent employment in this position you must be an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport.
