Responsibilities
  • Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
  • Manage client information in CRM & CMS ensuring all details and case notes are up to date.
  • Engage in ongoing care discussions, planning, assessments, reviews, and evaluating goals with clients and care teams.
  • Spend time on the road completing scheduled visits, interact one-on-one with clients within their homes, and support daily activities.
  • Provide care in accordance with each client\'s individualised Care Plan.
  • Support community-based recreational and social activities; provide transportation and assistance with shopping and appointments.
Role Requirements
  • Certificate III Aged Care, Individual Support or an equivalent qualification – Essential
  • Current Australian Driver\'s Licence (Green Provisional minimum) – Essential
  • Roadworthy & insured (Third Party Property minimum) vehicle to use for work – Essential
  • Experience with technology including smartphones, tablets, laptops, etc.
  • Confidence to satisfy the relevant probity checks required by legislation or policies
  • Ability to work with outcomes in consultation with the care team as required.
