Job Description
Hanrahan Health is expanding its services to include positive behaviour support We seek an experienced Behaviour Support Practitioner at the Proficient or Advanced level, dedicated to providing evidence-based, person-centred, and outcome-focused support to NDIS participants.
Daily responsibilities include:
- Assisting NDIS participants, their families, and carers with needs assessment and person-centred planning.
- Developing and delivering support plans within complex support settings.
- Creating meaningful goals and strategies based on positive behaviour support principles, aiming to eliminate restrictive practices in home and community settings.
- Aligning individual plans with participants' goals, passions, and strengths.
- Building capacity and knowledge among supporters, including family and carers.
- Collaborating within a multidisciplinary team and mentoring less experienced practitioners.

About you
- Current suitability at Proficient or Advanced level with documentary evidence.
- Extensive experience supporting individuals with behaviours of concern.
- Excellent report writing and communication skills.
- Valid driver's license and reliable vehicle.
- Genuine passion for disability support.
Qualifications
- Degree in an allied health discipline is preferred.
