The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The Aged Care and Disability Services team aims to provide support services to help Aboriginal and Torres Strait Islander people over the age of 50 or living with a significant and/or permanent disability to be more independent and to actively participate in their communities. We have locations across the Southeast Queensland region including, North Brisbane, South Brisbane, Caboolture, Sunshine Coast, Gympie and Hervey Bay/Maryborough.
About the Role?
The position will implement and provide a program of respite services support activities to designated elderly Aboriginal and Torres Strait Islander people (and their carers), in line with Client Care Plans. The position will contribute to program development and review, ensuring elderly people have access to the most effective mix of services available, and that service provision is adequately coordinated and meets program objectives.
Within this role, you will be required to:
• Develop a Work Plan that details the steps and actions required to support a program of respite services to clients.
• Develop and implement programs to develop skills and abilities so they are able to make decisions affecting their lives, live as independently as possible and to help them participate in the community
• Help clients develop and maintain independence and safety in personal care, health care and hygiene (such as eating, dressing, toileting and bathing)
• Participate and in some cases lead, personalised structured activities, group activities or small group outings
• Contribute to the development of evidence-based programs to meet the needs of aged-care clients
• Contribute and participate in the installation of best practice in service delivery in respite care services that support the planning, implementation and review/evaluations of respite care services and programs.
About You?
- Current C Class Drivers Licence (QLD)
- Current Criminal History Check – required prior to your commencement
- Current Disability Worker Screening Check (Yellow Card) – required prior to your commencement
- Current First Aid and CPR Certificate
- Minimum Certificate in Aged Care or relevant qualification - required prior to your commencement
- Preferred Requirement to have (or willingness to obtain) a Light Rigid (LR) driver's licence
Desirable, but not mandatory:
- Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
- Knowledge of health and well-being issues affecting Aboriginal and Torres Strait Islander people and a strong commitment to improving well-being outcomes
