The Role Reporting to the Operations Manager, you'll play a pivotal role by delivering exceptional person-centred caremanagement to our elderly clients in their own home.
You'll be more than just a casemanager - you'll be an advocate, problem-solver, and partner in helping our elderly clients live fulfilling, independent lives.
What You'll Be Doing Work closely with home care package recipients to design care plans thatprioritise independence, well-being, and dignity.
Collaborate with our internal teams to ensure the seamless delivery of high-quality care.
Connect with clients, families, and stakeholders to foster trust, engagement, andexceptional outcomes.
Identify risk factors, engage in a co-management approach to care planningensuring appropriate referral of clients to specialised services.
Ensure integrity of client related documentation and data entry, maintainingaccurate and timely records in Sai's client care managementsystems Who You Are You have a background in case management and aged care and are dedicated to making a difference.
Bonus if you have demonstrated aged care experience as an Enrolled/Registered Nurse Thrive in a fast-paced, agile environment and love problem-solving and takinginitiative.
Your ability to engage, advocate, and inspire shines through in everything you do.

A valid Driver's Licence and a reliable vehicle are required for this role.
Regulartravel within the South-East region and Mornington Peninsula.
