About the Role The HCP Care Partner role focuses on delivering outstanding person-centred support to our clients.
By building meaningful relationships with internal and external stakeholders, this role ensures clients receive the care and support they need to live the life they choose.
Key responsibilities include: Coordinating services within business guidelines Implementing client service responsibilities aligned with client needs Ensuring compliance with Aged Care Quality Standards and HCP requirements Conducting assessments, reviews, and intake of new clients The role is based out of our Tullamarine office with WFH flexibility and travel to visit clients as required.
About You You are a supportive, friendly, and organised individual with excellent time management, leadership, and customer service skills.
You thrive in a dynamic and innovative environment and enjoy being part of a highly skilled team committed to quality care.
