Working as part of a multidisciplinary and multiagency team, you will deliver improved mental health (MH) and alcohol and other drugs (AOD) services, you will be responsible for providing a range of clinical services including intake, assessment, therapeutic intervention, treatment coordination, care planning, case management, group work, and discharge planning.

  • The Regional Care Partnership, led by Ballarat Community Health, is a team of highly experienced health care agencies tasked with delivering improved mental health and alcohol and other drugs (AOD) services to the Ballarat Goldfields region on behalf of Western Victoria Primary Health Network (WVPHN).


The Partnership will target the individual needs of health care clients who are seeking a holistic approach to their health support.

The Partnerships model is about care and coordination.

Each client will be supported by a team of health care providers with specialist skills and knowledge to lead them through the journey to better health.


Services will be delivered in a person-centred, integrated stepped care framework, targeting the individual needs of clients who were seeking a holistic approach to their health care.

Clients, and their families and carers, will be supported by a multidisciplinary team who will use their specialist skills, lived experience and knowledge providing streamlined referrals, better access to community networks, mental health counselling, care coordination, one-on-one outreach support and group work programs to people in an inclusive, culturally safe environment.


What you will accomplish:

  • Deliver comprehensive flexible therapeutic support services including assessments, brief interventions, treatment planning, evaluations, and education in line with professional, ethical and National Standards.
  • Provide evidencebased mental health interventions, allied health assessments, short term interventions, and behavior support plans.
  • Provide high quality care coordination and advocacy to meet the specific needs of individuals.

What you will bring to the team:

  • Tertiary degree qualification in Social Work, Psychology, Mental Health, Alcohol and other Drugs, Occupational Therapy, Nursing, or other health related field and hold current registration with AHPRA, AASW and /or Mental Health accreditation.
  • Expertise in working with clients with mental health issues, AOD issues, complex needs and interacting with families and carers is desirable.
  • Capability to implement therapeutic, recoveryoriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
