This role is responsible for:

  • Conducting member's care assessments and care planning
  • Managing members compliance.
  • Intake processing, clinical care planning and reporting
  • Coordinate the provision of quality person centred care according to the member's needs, service/care plans and available funding in accordance with budget
  • General administration with respect to tasks performed
  • To relieve the Roster/ Recruitment or EN/SW Coordinator when on leave if the other are unavailable
  • To relieve HR/ Training Coordinator when on leave with support worker inductions.
  • To relieve (on call) for the EN/SW Roster Coordinator when on leave.
  • To relieve/ back up reception when on leave.
Preferred Skills

Preferred Skills:

  • Relevant experience in coordinating responsive, individualised support to participants of the NDIS, including individuals with complex needs
  • Enrolled Nurse or Registered Nurse will be an advantage
  • First Aid & CPR are essential
  • NDIS Worker Screening check
  • Valid Driver's Licence
  • Medication Management Training
  • NDIS worker orientation online training.
  • NDIS COVID19 Online Training.
  • Knowledge of rostering and highly developed organisational skills with an ability to meet crucial deadlines.
  • Be available to work flexible hours.
  • Experienced in assessing and planning care needs within the community
  • High level of computer literacy, with a sound understanding of Microsoft Office
  • An outgoing personality with high energy levels, good communication skills and personable telephone manner
  • Ability to problem solve and think quickly under pressure
  • Possess a 'cando' positive attitude and work well in a small team environment
  • Ability to multitask with a strong attention to detail while keeping calm and focused with a positive attitude to people with disabilities.
  • Excellent networking, consultation skills to develop new referral sources and relationships with appropriate external organisations
  • Excellent customer service skills with exceptional verbal and written communication skills
  • Sound knowledge and understanding of home care services and guidelines
  • Excellent time management skills
  • Understanding of and commitment to Occupational Safety and Health Policy, Procedures and Guidelines and Equal Opportunity Principles and Practices.

Job Types:

Part-time hours: 24 per week

Salary:
From $25.00 per hour

Schedule:

  • 8 hour shift
  • Day shift

COVID-19 considerations:
Requirements are to maintain both yearly Flu and Covid vaccines, utilise PPE and follow Dept of Health guidelines.
