About The Position
This is an exciting opportunity for a dedicated Case Manager (Clinical) to make a meaningful impact in the lives of residents in our communities. As a Case Manager (Clinical), you will play a pivotal role in supporting integrated and coordinated resident care and service requirements. You'll be responsible for pre-entry assessment, move-in support, ongoing assessment, care planning, accessing externally funded care, care delivery and evaluation, referral management, social support, and networking.

Reporting to the Regional Manager – Care, you will work across our Bartonvale Lodge and Torrens Grove Retirement Villages. Your key responsibilities will include:

  • Conduct pre-entry assessments for incoming residents.
  • Provide move-in support to ensure a seamless transition.
  • Conduct ongoing assessments to monitor resident needs and preferences.
  • Develop and implement individualised care plans.
  • Assist residents in accessing externally funded care resources.
  • Coordinate and oversee the delivery of care services.
  • Manage referrals to external healthcare providers and services.
  • Facilitate social support and networking opportunities for residents.
  • Monitor the work practices of Care Services Workers and provide direction and guidance as required.

Essential Criteria
As part of the application process, you will be required to provide a National Police Check, proof of right to work in Australia, verification of identification and qualifications, and participate in pre-employment assessment.

  • Demonstrated experience in similar role and industry.
  • Diploma of Nursing (including registration).
  • Drivers Licence and Reliable Transport.
  • National Police Check (or ability to obtain).
  • First Aid & CPR Certificate (or ability to obtain).
  • Prepared to undertake a pre-employment medical assessment.
