What you will be doing:

  • The purpose of the Paediatric Diabetes Social Worker is to ensure optimal outcomes for children and their carers referred to the Paediatric Diabetes Clinic. In collaboration with paediatricians, endocrinologists and senior nurses the Paediatric Diabetes Social Worker will help to identify the psychosocial needs of families presenting to the Clinic. Additionally, the Clinic Social Worker provides a range of social work interventions, to ensure continuity of care, safety, and transition to adult services as needed.

Selection criteria to be addressed:

  • A Social Work Degree accredited by the Australian Association of Social Workers
  • Capacity to exercise independent professional judgement on routine matters and seek professional mentoring in relation to performing novel, complex or critical tasks
  • Practise with a high level of cultural competence and an ability to build effective relationships with other relevant government, nongovernment and community organisations.
  • Skills and experience in working with children and families
  • An understanding of the psychosocial consequences of living with diabetes
  • An understanding of quality improvement processes
  • High level written and verbal communication skills
  • An understanding of child protection principles and statutory requirements
