Care Manager


Overview


We are seeking somebody for a Permanent Part-Time position initially, with an FTE of We will also consider a dual contract for a support worker role for those seeking additional hours.


Scope of Responsibilities
As a Care Manager at Flourish Support Services, you will be entrusted with a multifaceted range of duties that are crucial to our daily operations:


  • Participant Relationship Management: Maintain impeccable communication and cultivate positive relationships with all participants.

Clinical Oversight: Collaborate on the development and execution of customized care plans, supervising the provision of services by support workers.

Client Communication: Promptly respond to client inquiries and concerns.

Staff Development: Spearhead training modules and ongoing developmental programs for support workers.

Client Onboarding: Implement comprehensive procedures for welcoming new participants to our services.
-
Documentation: Assiduously manage client files to ensure all records are up-to-date and comply with regulations.

Stakeholder Collaboration: Work closely with external stakeholders such as healthcare providers, insurers, and regulatory bodies.
-
Operational Flexibility: Assume rotating on-call responsibilities as part of our continuity of care commitment.

Requirements:

Essential Requirements:

  • Valid Paid Blue Card or willingness to obtain
  • NDIS Worker Card or willingness to obtain
  • NDIS Worker Orientation Module or willingness to obtain
  • COVID19 Infection Control Certificate or willingness to obtain
  • Monday-Friday, 8:00am 4:00pm availability
  • Ability to participate in rotating oncall responsibilities
  • Proficient in computer operations
  • Exceptional problemsolving capabilities
  • A valid driver's license and registered vehicle

Desired Qualifications:

  • Diploma in Community Services or equivalent
  • Minimum 2 years of industryrelevant experience
  • Proficient understanding of NDIS regulations and practices
