Role Overview

As an Assistant Support Facilitator, your primary responsibility will be to support Real Community Service's Facilitators across South-East Queensland in an administrative capacity. Your role is crucial in delivering timely, sustainable, and person-centered support to individuals with disabilities, helping them achieve their goals and maintain meaningful relationships. This newly established position offers a fantastic opportunity for career growth, gaining valuable experience, and making a real difference in people's lives. This role will be based at our Maroochydore Office with frequent travel to the Gold Coast. You will receive support from the Regional Manager and ongoing training.

Main Responsibilities

  • Assist Facilitators in developing and managing Participant profiles to ensure compliance with NDIS Practice Standards.
  • Collaborate with Facilitators to create personalized support plans for Participants.
  • Support in risk assessment and management to ensure safety for Participants and Staff.
  • Monitor and document Participants' progress, provide feedback, and suggest improvements.
  • Handle daily administrative tasks such as phone calls, document collection, and file management.
  • Coordinate daily activities and ensure effective delivery of support services.
  • Create a positive and welcoming environment for Participants and Staff.
  • Participate in team meetings and contribute to service enhancements.

Desired Skills and Qualifications

  • A genuine desire to help and positively impact the community.
  • Understanding of the National Disability Insurance Scheme and its implications.
  • Strong communication and interpersonal skills.
  • Proficiency in computer applications and numeracy.
  • Excellent organizational skills and attention to detail.
  • Ability to work autonomously and collaboratively.
  • Flexibility to meet participants' diverse needs.
  • Prior experience in support or caregiving roles is beneficial.
  • Preferred qualifications in Disability or Health Care.

Required Certifications and Skills

  • Blue and Yellow Card/NDIS Card.
  • NDIS Worker Orientation Certificate.
  • Valid QLD driver's license.
  • First Aid and CPR certifications.
  • Qualifications in Disability or Health Care field are advantageous.
  • Experience in Case Management is a plus.
