About the role


Responsible for the provision of a therapy and practical support with activities of daily living as directed by the Multidisciplinary team (MDT).

Focussed on meeting the needs of patients and their families who are patients of the CALHN GITH program.

You will contribute to the experience of older persons during their transition from the acute care hospital environment back to their home or via direct admission as part of hospital avoidance/ substitution strategies.

As a member of the team, the AHA will work under the direction of the MDT

Educational/Vocational Qualifications
Certificate III in Allied Health Assistant or equivalent Certificate in Aged Care or Community Health and/or Disability or other health related qualification
