About the Role - Team Leader - Care Management


We have a great opportunity for an experienced, highly motivated Team Leader to work in our Aged Care team in North West suburbs or surrounds, VIC in a Full-time role.

The role provides leadership to client service staff (primarily Care Managers) to deliver safe, effective, personal and connected care for Aged Care clients in line with Aged Care legislation, program guidelines and standards.

The role operates across a geographic footprint designed to deliver on the objectives of the Area Model which delivers a high quality and consistent client experience at a local level.


In this role you will:

  • Provides reporting and real time updates to Regional Manager Aged Care and other team members across the organisation in order to track and improve performance within agreed time frames and KPIs
  • Lead a culture of agility and innovation understand, articulate and drive continuous improvement as well as transformation
  • Provides recommendations to Regional Manager Aged Care and the Regional Business Development Manager in relation to business development and supports the implementation of local business development opportunities
  • Coach and develops to build high performing and resilient teams / individuals in the context of a highly mobile workforce

Success in this role
You have a minimum of 5 years' experience in leading a case manager within Aged Care. You love helping others, you are confident in delivering leadership to client service staff. You are committed to treating others with respect and maintaining confidentiality. You share our workplace values of Collaboration and Courage, Authenticity, Respect and Excellence - CARE.

You have

  • Evidence of full Covid19 vaccinations
  • A diploma or degree in Health or Allied Health field or minimum of 6 years' relevant experience to equate to a healthrelated discipline
  • Leadership experience (Leadership qualification desired but not essential)
  • Strong communication, advocacy and interpersonal skills
  • Demonstrated time management, prioritisation and budget management skills
  • Demonstrated experience in the provision of case management
  • Current Australian Driver's Licence and access to a reliable vehicle
  • Experience in the Microsoft Office suite and exposure and/or ability to acquire skills for inhouse client database systems
