We are seeking a compassionate and dedicated Dementia Support Worker to join our team.

The primary role of the Dementia Support Worker is to provide high-quality care and support to individuals living with dementia, facilitating their engagement in activities and promoting their overall well-being.

The position operates within the framework of the NDIS guidelines to ensure the provision of person-centered, goal-oriented support.

Key Responsibilities:

  • Provide oneonone support to individuals living with dementia, both in their homes and in community settings.
  • Assist clients with personal care activities, including bathing, dressing, and grooming.
  • Foster a safe and supportive environment for clients, promoting their independence and dignity.
  • Develop and implement individualised care plans based on clients specific needs and goals.
  • Engage clients in meaningful activities, such as social outings, cognitive exercises, and recreational pursuits.
  • Monitor and document clients' progress, noting any changes or concerns in their condition.
  • Collaborate with clients' families, healthcare professionals, and other support workers to ensure a coordinated approach to care.
  • Provide emotional support and companionship to clients, promoting social interaction and mental wellbeing.
  • Maintain accurate records and documentation in accordance with NDIS requirements.

Qualifications and Experience:

  • Certificate III or IV in Individual Support/Aged Care/Disability or a related field.
  • Previous experience in providing care and support to individuals with dementia is highly desirable.
  • Familiarity with the NDIS framework and guidelines is an advantage.
  • Strong interpersonal and communication skills, with the ability to build rapport with clients and their families.
  • Compassionate and patient approach, with the ability to empathise with individuals living with dementia.
  • Ability to work independently, prioritise tasks, and adapt to changing circumstances.
  • Understanding of dementiarelated behaviors and the ability to effectively manage challenging situations.
  • Valid First Aid and CPR certifications
  • Current driver's license and access to a reliable vehicle
  • Valid Police Check
