About the role

Reporting to the Manager In Home Support, the Team Leader Home Care Victoria works within the In Home Support division overseeing the provision of care and services, including but not limited to intake, assessment, care coordination and management, to Elders living in the community and providing direct supervision to staff in the Care Team.

  • Pivotal in driving service operations and business growth, providing leadership and supporting change management and continuous improvement in line with ongoing aged care reforms.
  • Provide leadership and direction to all staff to achieve high quality services in line with the Aged Care standards and MiCare's vision and mission
  • Ensure services are delivered under the principles of Consumer Directed Care and appropriately developed care plans that support assessed client goals, diverse needs, wellness and reablement principles within allocated package budgets.
  • Participate in the evaluation and ongoing review of Home Care Services ensuring high quality and cost-effective services.
  • Drive the maintenance of the accreditation status for Home Care in accordance with the Aged Care Quality Standards.
  • Promote Home Care within the community to develop the growth of the service by expanding and building networks and presenting MiCare's services and brand to the community to attract consumers.

About you

  • Tertiary qualified in a relevant discipline
  • Proven ability to effectively support and lead case management and support staff
  • Demonstrated knowledge and sound case management experience in the aged care sector
  • Self-motivated with exceptional interpersonal skills
  • Experience in supporting teams through change management processes
