Role Overview:

This is an exciting opportunity to join UnitingSA's Community Aged Care team as a HomeCare Support Worker. Deliver tailored care promoting safe, community living in line with Care Plans. Document provided support and promptly escalate any health concerns to ensure wellness and safety.

Key Responsibilities:

    • Deliver personalised, goal-oriented care for independent living at home and in the community.
    • Escalate client-related issues promptly to a Coordinator or after-hours On Call.
    • Communicate openly and sensitively with clients, families, and advocates.
    • Actively participate in directed individual and group care activities.
    • Guide clients to their Coordinator for service changes or additional support.
    • Provide tailored support to culturally diverse clients.
    • Assist clients in communicating service changes to their Coordinator.
    • Monitor and swiftly report changes in clients' well-being to a Coordinator or after-hours On Call.

Skills & Experience:

  • Minimum of 1 year of experience working in a community/aged care setting (Essential).
  • Proven ability to effectively communicate with clients, their relatives, carers, and/or advocates, as well as substitute decision-makers.
  • Experience in catering to the needs of older individuals from culturally and linguistically diverse backgrounds (CALD) – desirable.
  • Demonstrated capability to work autonomously with minimal direct supervision.
  • Proficiency in comprehending and adhering to client Care Plans.
  • Track record of establishing rapport with clients, their families/carers, key stakeholders, and fellow staff.
  • Minimum qualification of Certificate III in Individual Support or Community Services – essential.
