The role you could play
You will provide intervention through exploration of the impact of substance use on the individual and their families. You will be responsible to provide care and recovery coordination around issues caused by substance use.
This is instrumental to provide liaison and support through interventions to assist in the health & wellbeing for individuals experiencing alcohol and other drug related harms to clients referred for Care & recovery Coordination and Counselling to The ECADS program.
Duties- Service providers have up to date service and referral pathway information
- Case notes completed within 2 working days of each counselling session, reflects appropriate treatment aligned with client complexity
- Documented evidence of community focused supports for each client
- Recovery Plan commentary reflect liaison with relevant agencies
- Client engagement data entered & maintained on the PENELOPE & SAMIS system within 2 working days of contact with client
- Relevant qualification in Social Sciences or other relevant discipline or equivalent experience in a social program environment
- HLTAID009 Provide cardiopulmonary resuscitation
- HLTAID010 Provide basic emergency life support
- HLTAID011 Provide First Aid
- Understanding of the Community Services sector
- Experience in basic research and analysis and preparation of documentation
- Solid knowledge and capability with technology including data bases
- Commitment to the mission and values of The Salvation Army
- Current VIC Driver's Licence
