As part of our team in Hamilton North, we are looking for a Support Worker to work casually across both our Social Activity Group and Home Care Packages programs.
- Social Activity Group responsibilities:_
- Assist and work alongside older people whilst in a supported group to enable their identified needs and goals to be fulfilled.
- Encourage and support people to actively participate in planning and feedback forums when making decisions about the service they receive, and ensure activities are responsive to this feedback.
- Regularly engage with clients in groups on outings or in our centre as they challenge their minds in a variety of activities or even at a simple gathering sharing stories.
- Home Care Packages _responsibilities:_
- Deliver highquality individualised support and assist our older adult clients, supporting them to maintain and develop their independence so that they can lead enriching and fulfilling lives.
- Follow and implement individual service plans, healthcare, and positive behaviour support plans.
- Provide companionship, conversation, and emotional support and support with personal care in the client's home.
- Assisting with transportation to attend appointments.
Qualifications:
If you have experience working as a _Support Worker_, are qualified with a Cert III or IV in Aged Care, Community Support or related field, and enjoy variety in your day, you might be exactly the person we're looking for
- Qualification in Cert III or IV in Aged Care, Community Support or related field.
- Experience working with older adults.
- The ability to follow and implement individual service plans, healthcare, and positive behaviour support plans.
- A desire to use your skills and experience creating fun and interesting activities that support older adults to engage more consistently with their community and peers.
- A valid driver's licence.
