DEPARTMENT
Providing responsive and sustainable care, the Advance Care Planning (ACP) program at Alfred Health promotes every person's right to autonomy, dignity and respect through full informed consent and shared decision making in relation to their current and future healthcare treatment

The ACP program supports:

  • Individuals through a process of planning for future health care through Advanced Care Planning
  • Medical Treatment Planning and Decisions Act 201
  • Improving End of Life Care
  • National Safety and Quality Health Service Standards
  • Individual's wishing to access the Voluntary Assisted Dying
  • Voluntary Assisted Dying Act 2017
***The Advance Care Planning and Voluntary Assisted Dying Clinician will work across and support the three streams of the program

The role will:

  • Provide a highquality ACP service to inpatients and outpatients referred to the ACP program
  • Be responsible for assisting patients with ACP discussion and the completion of ACP documents
  • Support clinical staff to have ACP conversations with their patients and providing training and mentoring to enable these discussions to occur
  • Be expected to work across all three hospital sites and visit the local community health centres and other community facilities (e.g. residential care facilities) as required
  • Be responsible for responding to voluntary assisted dying referrals as guided by the Voluntary Assisted Dying Act and the Alfred Health Voluntary Assisted Dying Guideline
  • Will be responsible for managing voluntary assisted dying enquires for coordinating and consulting medical practitioners from patients, GP's and Statewide Care Navigators in collaboration with Medical Services

QUALIFICATIONS/EXPERIENCE REQUIRED

Essential

  • Experienced Health Clinician with tertiary qualification in relevant discipline, meets registration requirements with Australian Health Practitioner Regulation Agency (AHPRA) for registered professions.
  • Demonstrated understanding of clinical governance and patientcentred care
  • Demonstrated ability to work autonomously, and highly motivated to drive the success of this project
  • Demonstrated ability to engage effectively with key stakeholders, and foster teamwork and collaboration
  • Reliable and highly organised

Desirable

  • Experience in palliative care, endoflife care, Advance Care Planning, and or Voluntary Assisted Dying
  • Previous demonstrated end to end quality improvement experience with complex projects that impact on multiple departments and stakeholders
  • Previous demonstrated experience in research
