Job Title: Advanced Practice Nurse - Palliative Care

Job Summary:

We are seeking an experienced and skilled Advanced Practice Nurse to join our Palliative Care team at St Vincent's Health Australia. The successful candidate will be responsible for providing high-quality patient care, leading clinical teams, and contributing to the development of palliative care services.

Key Responsibilities:

  • Provide clinical leadership and guidance to nursing staff in the delivery of palliative care services.
  • Assess and review patient and family care needs, developing and implementing individualized care plans.
  • Contribute to the development and implementation of palliative care policies and procedures.
  • Collaborate with multidisciplinary teams to ensure seamless care transitions and coordination.
  • Engage in education and training activities to promote best practice in palliative care.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.

Requirements:

  • Registered Nurse Practitioner with the Nursing and Midwifery Board of Australia and AHPRA registered.
  • Demonstrated experience practicing at an advanced clinical practice level in palliative care.
  • Ability to work autonomously and collaboratively as part of a multidisciplinary team.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
  • Knowledge of palliative care principles, practices, and policies.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
