Deivian Care is looking for Support Workers in Burwood, Surrey Hills and Wheelers Hill (VIC) and surrounding areas with expertise helping vulnerable and marginalized persons through high-quality case management, social work, and outreach. You will play an important role for many participants who may be excluded from mainstream healthcare, social support, and opportunities, negatively impacting their functioning and independence in the community. You'll use evidence-based, integrated, and person-centered approaches to give individualized assistance using clinical frameworks and modalities to create co-developed realistic recovery programs that are both individualized and powerful. Your goal is to help and encourage people to recognize their own self-efficacy and make long-term changes by taking control of their own journey.

Domestic cleaning, meal prep, Community access, Transport to and from medical appointments, Personal care, Manual Handling/ lifters and wheelchair transfers

Experience/Certification Required
- Relevant qualification Certification III or IV in Disability/Aged Care/Individual Support/Social Work
- Full Victorian Driver's License and your own vehicle
- National Criminal Check
- Working with Children Check
- First Aid & CPR (or willingness to get one)
- Covid19 Vaccination Certificate
- Manual Handling (not mandatory but preferable)
