Your Opportunity
To provide case manage to Homeless families and young people that are at risk of homelessness.

Your key responsibilities will be to:

  • You will be required to provide intensive ongoing person
- centred case management (formal and informal) in line with MA'S National Case Management Approach with complex clients and review progression against case plan.

  • Required to provide evidenceinformed case management sessions with clients that are sensitive and responsive to, the cultural strengths and needs of each client.
  • Assist client with exiting planning and the transition out of service into independence or other services including the developing the capacity to selfmanage and access required support independently.
  • Undertake general direction proficiently administrative skills including data entry for case management, referrals, case plans, support letters and other relevant administrative tasks.
  • Build and maintain positive internal and external relationships, with the ability to adapt communication style to resolve conflict including community, government nongovernment services, Indigenous organisations, clients, and their family/care givers.

Requirements for Success

  • Relevant Degree with 2 years' experience.
  • Alternatively, may hold diploma with relevant working experience or a less formal qualification with substantial years of relevant experience gained through previous industry experience.
  • Demonstrate professional approach to the role maintaining professional boundaries.
  • Well developed written and verbal communication skills including well developed administration and computer skills and the ability to provide support management of complex nature.
  • Demonstrate ability to provide interventions and manage a case load of complex clients.
  • Positive and personcentred approach with strong guiding belief about everyone's capacity to selfright and grow within and beyond their current circumstances.
  • Current Drivers Licence.
  • Valid Working with Children's Check.
