The purpose of this role is to demonstrate accountability at an advanced practice level for managing and coordinating ongoing care for consumers/person with multiple presentations to the TPCH Adults Emergency Department (AED) or the Children's Emergency Department (CED). for the provision of specialist nursing advice, consultation, leadership and education to inter-professional team members as relevant to the specific context of the role.

The role will:

  • Coordinate consumer/patient centred care across the consumer/patient journey.
  • Create partnerships, for example, providing a central point of communication, engagement and coordination for all stakeholders involved in the consumers/patients care.
  • Improve consumer/patient outcomes through the delivery of evidence-based care.
  • Facilitate systems improvement - providing a leadership role within the organisation and act as an agent of change.

About you

This successful applicant will carry out the following key accountabilities in accordance with the Metro North values and the corresponding Lominger competencies:

  • Fulfil the responsibilities of this role in accordance with Schedule 2, Nurses and Midwives (Queensland Health Award – Generic Level Statements (GLS) Nurse Grade 7 and the Queensland Health and Metro North Health's commitments and values, in achievement of the organisational goals and Domains of Practice.
  • Practice in accordance with code of ethics, professional standards and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.
  • Demonstrate comprehensive knowledge, and the ability to apply relevant legislation, guidelines and standards.
  • Provide leadership and direction within the AED and the CED, which facilitates a consistently high level of nursing care.
