About the Team:

6 South West is a 28-bed acute medical/surgical unit with a passionate team of over 60 nurses, specialist allied health teams, and experienced consultants caring for Nephrology patients through the spectrum of renal disease.

A collaborative approach to care by the multidisciplinary team fosters an environment where nurse input is valued for optimal patient outcomes.

Your new role:

  • Provide high-quality patient care through assessment, planning, implementation, and evaluation in collaboration with the multidisciplinary team.
  • Liaise with the healthcare team to achieve desired patient outcomes.
  • Maintain responsibility for patient safety, quality, and risk while contributing to organizational quality and safety initiatives.
  • Perform nursing interventions and procedures according to Melbourne Health policies.
  • Deliver evidence-based nursing care.
  • Contribute to an effective discharge plan, provide patient education, and make referrals to community providers.
  • Demonstrate competency in clinical skills.

About you:

  • AHPRA registered Nurse.
  • Effective communication skills and a passion for continuous professional development.
  • Readiness for new clinical experiences to enhance professional growth.

Desirable

  • Post Graduate Certificate in Renal Nursing.
  • Recent clinical experience in acute medical or surgical settings.
