Our Southern Cross Care Health and Wellness team at Oakfield Rise is looking for a Health and Wellness Promoter (Exercise Instructor) passionate about helping empower our clients towards better quality of life through tailored health and exercise programs.


About this role


You will work under our tested multidisciplinary model of care that has significant, proven benefits for getting our clients onto their best health pathway.

Our model uses a comprehensive assessment battery and case conferencing process to inform targeted prescription, with the client journey culminating in ongoing exercise and service participation.


Key responsibilities of this position would include:

  • delivery of group based and 1:1 exercise programs under the guidance of an Exercise Physiologist
- contributing positively to multi-disciplinary care planning for our clients
- demonstrating experience in person-centred practice, service continuous improvement, evidence based treatment and service delivery
- proactively delivering evidence-based reablement pathways that ensure client physical, social and mental health benefits
- demonstrating willingness to work as part of a team and to increase your personal skill set
