About the role


We are seeking a dedicated Case Manager to provide case management client support to single women over the age of 25 with accompanying children at risk of homelessness.

This position aims to address barriers and provide access pathways into stable housing - the Mothers Emergency Accommodation program offers a range of crisis/emergency support including 13 night accommodation / 6 -12 weeks emergency accommodation and outreach support.

Responsibilities Include

  • Establish effective relationships with clients to encourage selfdetermination
  • Provide support accessing stable housing including crisis accommodation
  • Provide support to those who have experienced family violence
  • Conduct assessments from initial intake to exit outlining client's issues
  • Evaluate the wellbeing of children with focus on safety, health, education
  • Conduct wellbeing assessments to assess children's needs and set goals
  • Regularly review progress of individual case plans to ensure action is taken
  • Develop strong professional relationships with Department Families, Fairness and Housing (DFFH), allied services, The Orange Door, and Child Protection
This is a permanent part-time position, based in Ballarat, VIC. Salary and conditions are in accordance with SCHADS Level 5.

What you will have

  • Tertiary qualification in Social Work/Welfare studies or equivalent in Human Services Field, (a degree qualification is preferred)
  • Relevant experience in a Social Service environment
  • Supports others in challenging situations and acknowledges outcomes
  • Clearly explains and presents ideas and arguments
  • National police record check is required
  • Current and valid Working with Children Check
  • Current Victorian Driver's Licence
