The Position

As a Client Outcome Facilitator, you will be responsible for supporting clients at our largest immediate supported accommodation facility in Spring Hill. Your role involves providing holistic support throughout the client journey and empowering clients towards independence.

An important aspect of this role is assisting clients in reconnecting with their community by offering information and referral guidance as part of approved case management plans.

Please note - we are seeking an individual for night shift on a fixed-term (until 30/06/2026) full-time (38 hours per week) contract located in Spring Hill. While this role is currently for night shifts only, you may be required to work different shifts, including afternoons, nights, and weekends based on organizational needs, as outlined in the program Service Agreement.

To excel in this role, you should meet the following criteria:

  • Minimum Certificate III in Community Services or related field or Alcohol and Other Drug (AOD)
  • Knowledge of and ability to work within relevant legislation and guidelines related to case management and safety within the program's operational area
  • Prior experience as a Support Worker or similar role in the program's operational area
  • Ability to work within specified guidelines, follow policies and procedures diligently
  • Strong interpersonal, communication, and organizational skills, including sound judgment and teamwork capabilities
  • Capability to advocate for clients, maintain client information confidentiality, and uphold client confidentiality at all times
  • Understanding of and dedication to the values and mission of the St Vincent de Paul Society Queensland and alignment with policies and procedures to support the Society's mission
  • Verification of COVID-19 vaccination status is mandatory
