_The Role:
_


Provide crisis response and case management to clients who are experiencing domestic and family violence referred through Domestic Violence Rapid Response (DVRE) and Specialist Homelessness Services (SHS) (Assess and Connect).

Provide Domestic and Family violence risk and needs assessment (including DVSAT), safety planning, crisis support and case management to clients via telephone, at the organisations sites and in an outreach capacity.

Comprehensive, trauma-informed, strengths-based and person-centred assessment and case planning in adherence with the organisations policies, procedures and relevant legislation.

Provide appropriate referrals to internal and external services, client advocacy and maintain relationships with key stakeholders to support client outcomes.

Facilitate case co-ordination internally and externally with key DFV partners such as Department of Communities and Justice and NSW Police.

Conduct formal and informal client reviews to evaluate strengths, progress and emerging needs and goals. Perform after hours On-Call duties as part of a rotating roster. Produce high quality client record-keeping including case notes, data, documentation and report writing as required.

  • Qualifications and/or equivalent experience in the areas of youth work, social work, education, psychology, community services or other relevant fields.
  • Working with Children Check
  • NSW Drivers licence
  • Police Check.
