About the role
As our Support Coordinator, you will support people with a disability and their family or carers to design, coordinate and monitor the delivery of services to enhance an individual's quality of life.
Use strengths based practice to support your clients to identify their specific needs, preferences and goals; to promote their independence and participation in the community.
Duties
- Coordinate and link clients with services in line with their NDIS plan and disability needs
- Establish and maintain network relationships with key agencies, other service providers, and referral services.
- Provide information and referrals to support clients to access appropriate resources and services, both within The Benevolent Society and with external agencies and service providers.
- Monitor the budget of the NDIS plan and allocations to service providers
- Build participants capacity to understand their NDIS plan and how the plan can support their needs

Skills and Experience

Relevant qualification and/or minimum 1 year experience in a Support Coordination role:
- Demonstrated understanding of the needs of people with a disability and their carers
- Strong evidence of relationship building skills
- Strong evidence of communication and report writing skills
- Excellent interpersonal and communication skills and a strong commitment to customer service
- Current driver's license and willingness to travel to clients where required
