In line with your commitment to customer care, you will work with older adults and their support network to deliver the best individual supports.


As _Case Manager_ your duties will include:

  • Making use of your case management abilities to do initial and ongoing thorough assessments and care planning to support customers at home.
  • Managing the delivery of services to support the needs identified (psychosocial, medical, physical and vulnerabilities) within the customer' individualised budget.
  • Drawing upon and strengthening relationships with colleagues, services networks and a range of community agencies to support the program and customers.
  • Mentoring Support Workers within the HCP team.


On a typical day you will visit an older person in their home, follow up on their care requirements; attend a case conference; liaise with our Clinical Case Manager in relation to customer's clinical needs; attend to case management for your portfolio and provide reporting for governance and compliance requirements; complete rostering requests with a focus on customer needs and their financial package etc.


Qualifications:

If you hold a qualification in Aged Care, Health, Community Services or related discipline and enjoy assisting older adults with their day to day needs through accessing quality care providers, you might be exactly the person we're looking for

  • Experience and Knowledge required for the job;_
  • Experience providing case management to older adults and their carers in a home and communitybased setting or similar.
  • Highly developed Assessment and Care Planning skills a with a focus in wellbeing.
  • Experience delivering highquality personcentred support and assistance to individuals that embraces their personal goals and choices, and respects cultural diversity.
  • Proven ability to complete administrative tasks to the high standard and detail required.
