About the Role:


The Intake & Assessment Specialist will engage with people and families at risk of, or experiencing homelessness, at all entry points of the service to undertake quality assessments of their needs, complexities, risk factors and potential pathways.

As a client's first point of contact with the service or the system, you will play a critical role in ensuring clients receive a person-centered response using trauma-informed, safety-first and Housing First practices.


Duties:

  • Be the first point of contact for referrals to the homelessness service
  • Conduct initial intake assessments
  • Develop tailored case plans
  • Provide a seamless and efficient intake and assessment process
  • Accurate data entry of client information

Skills & Experience:

  • Demonstrated experience working and communicating with clients with multiple and complex needs
  • Proven experience and knowledge in case management
  • Mental resilience, positive and openminded attitude
  • Ability to adapt to a change, work independently and part of a team
  • Experience in intake and assessment or social / community services background
  • Current National Police Check and Working with Children Check
