The opportunity with D&J Connect:

Support Officer (Disability/Psychosocial)
We are seeking qualified, experienced and compassionate Support Officer for NDIS participants.

Who are we looking for:

  • Onboarding new NDIS participants and developing Service Agreements to reflect the supports, funding and services requested.
  • Assisting participants with complex needs to initiate their NDIS plan and develop their skills and confidence to manage their plan independently.
  • Working with potential and new participants to act as the key liaison point for all related services chosen by the individual.
  • Working alongside mainstream health services including Mental Health and
Disability-related services and hospitals.

  • Conducting risk assessments and home visits, developing support plans for the participants, and setting up those supports and services accordingly.


Interface with NDIS and mainstream services in a way that demonstrates competency and clarity around navigating all aspects of NDIS.


  • Ensuring all services are booked and rostered for participants in a timely way.
  • Understanding of basic computer skills including Microsoft Office and client reporting system.
  • Willingness and ability to work within and contribute to the vision, mission, core values and the guiding principles of the organisation.
  • Knowledge and understanding of diversity, gender and social inclusion, particularly
Aboriginal culture.
